Customer Application

All JK Findings customers must submit a customer application, business registration, and reseller documentation (when applicable) before their account can be set up.

Step 1: Your Business Registration

As part of our customer application you will be required to submit your business registration. Customers may submit their business registration by email to credit@jkfindings.com, fax to 585-292-0774, or as a digital upload option in the customer application below.

Customers using the digital upload option should have a file in PDF, JPG, JPEG, PNG, DOC, or DOCX format ready before starting the customer application in step 3.

Step 2: State Reseller Information (USA customers only)

Customers in states listed below must provide the reseller documentation listed for their state. If you do not see your state listed, you may skip this step. Customers may submit their business registration by email to credit@jkfindings.com, fax to 585-292-0774, or as a digital upload option in the customer application below.

Step 3: JK Findings Customer Application

All customers must complete a Customer Application form to set up an account and place orders. Forms are available in several languages. Please submit only one form.
